If you schedule a visit, please download, print, and fill out the appropriate forms and bring them with you to your appointment. This is critical; you won't have time to complete the forms in the waiting room.

Please call to make an appointment. We generally schedule first visits to be 90 minutes in length. Followup visits are usually scheduled to be 45 minutes to an hour. Dr. Monette is often available during the noon hour for short visits.
